Through the Site, we offer an online technology platform (the "Platform") that enables users and members ("Members") to provide health history information and connect with independent healthcare professionals ("Treating Providers") in order to obtain medical and healthcare services.

By accessing or using the Site, you agree to be bound by these Terms and Conditions. If you do not agree, you may not use the Site or the Platform.

THE PLATFORM AND ANY SERVICES PROVIDED BY TREATING PROVIDERS THROUGH THE PLATFORM ARE NOT A SUBSTITUTE FOR IN-PERSON OR OTHER MEDICAL CARE YOU MAY NEED.

IF YOU ARE EXPERIENCING A MEDICAL EMERGENCY, CALL 911 OR SEEK IMMEDIATE EMERGENCY MEDICAL ATTENTION.

2. Healthcare Services

Each Treating Provider who delivers Services through RevoraMD is an independent healthcare professional and is solely responsible for the medical care, advice, diagnosis, treatment, and other professional services he or she provides to you.

RevoraMD does not practice medicine or any other licensed profession and does not interfere with the practice of medicine or any licensed profession by Treating Providers.

YOUR USE OF THE PLATFORM AND ANY SERVICES PROVIDED BY TREATING PROVIDERS IS AT YOUR OWN RISK.

3. Site Content

Except for information provided directly by a Treating Provider during your care, none of the content on the Site should be considered medical advice or a representation, endorsement, or warranty that any particular medication, treatment, service, or course of care is safe, appropriate, or effective for you.

4. Informed Consent

Telehealth is the delivery of medical and healthcare services using electronic communications when the patient and the healthcare professional are not in the same physical location.

THE MEDICAL CARE YOU RECEIVE THROUGH REVORAMD IS NOT A SUBSTITUTE FOR OTHER MEDICAL CARE YOU MAY NEED.

Potential telehealth risks include, but are not limited to:

• Insufficient or incomplete information for appropriate medical decision-making
• Delays due to technology, software, internet, or equipment failures
• Rare privacy or security failures involving personal health information

By accepting these Terms and Conditions, you acknowledge that you understand the potential benefits and limitations of telehealth, that privacy and security laws protecting health information apply to telehealth, and that you have reviewed RevoraMD's Privacy Policy.

5. Privacy

RevoraMD is committed to protecting your personal information and complying with applicable privacy and security laws, including HIPAA where applicable. Our policies are described in our Privacy Policy.

6. User Account

To use certain features of the Site, you may be required to create an account by providing your name, email address, password, and other requested information. You agree that all information you provide will be true, accurate, current, and complete. You may not share your password, use another person's account, or create more than one account for yourself except where expressly permitted.

7. Use by Minors

The Site and Platform may be used by minors only under the supervision and control of a parent or legal guardian, and only where permitted by law. For any user under the age of 18, the Member must be the user's parent or legal guardian.

8. Use by Healthcare Providers

Healthcare providers using the Site or Platform represent and warrant that they have obtained all necessary consents, permissions, and authorizations from the patient and, where applicable, from the patient's parent, legal guardian, or authorized representative.

9. Use by Caregivers

You may authorize a caregiver or other representative to use the Site or Platform on your behalf, where permitted.

10. Access Rights

We grant you a limited, non-exclusive, non-transferable, revocable right to access and use the Site and Platform solely for your personal, non-commercial use. You agree that you will not impersonate any person, use the Site in violation of any law, reverse engineer any part of the Site, introduce harmful code, or post unlawful or objectionable content.

11. Service Delivery & Processing Times

RevoraMD provides telehealth services electronically through its online platform and secure patient portal.

After completing registration, patients receive instructions for accessing their account and completing the necessary medical intake.

Medical reviews begin after the required patient information has been submitted.

Processing times may vary depending on provider availability, the completeness of submitted information, and whether additional medical evaluation is necessary.

Patients may receive updates regarding their treatment status through their patient portal or other authorized communication channels.

12. Fees, Purchases, and Refund Terms

You agree to pay all fees, charges, and applicable taxes associated with your Account, purchases, or Services.

Refund Terms:

• Before medical review: 100% refund may be available if cancellation is requested before the medical team reviews your submitted medical history
• If not medically qualified: 100% refund may be available if, after medical review, you do not qualify for the applicable program or prescribed treatment
• After prescription issuance: No refund will be issued once you qualify for treatment and a prescription has been written and transmitted to a pharmacy

13. Subscription Cancellation Policy

At RevoraMD, we strive to make managing your treatment plan simple and convenient. Patients may pause or cancel their treatment plan at any time through either of the following methods:

1. Through Your Patient Portal

   Patients can log into their patient portal and navigate to the "Modify Treatment" section. From there, patients have the option to pause or cancel their treatment plan directly.

2. Through Chat & Support

   Patients may also request cancellation by contacting our "Chat & Support" team through their patient portal. Our support agents are available 24 hours a day, 7 days a week, and will process cancellation requests immediately upon receipt.

We are committed to providing a transparent and hassle-free cancellation experience. Patients are not required to call or provide a reason for cancellation.

14. Website Links

The Site may contain links to third-party websites, services, or resources. RevoraMD is not responsible for the availability, accuracy, content, or practices of any third-party website or service.

15. Ownership of Content and Trademarks

The Site and its contents, features, functionality, and design are owned by RevoraMD, its licensors, or other content providers and are protected by applicable intellectual property laws. You may not reproduce, distribute, modify, or create derivative works of Site materials except as ordinarily permitted through normal use.

16. Termination

You may deactivate your Account at any time by contacting us at support@revoramd.com. RevoraMD may suspend or terminate your access at any time, for any reason, subject to applicable law.

17. Right to Modify

We reserve the right to change, add, or remove portions of these Terms and Conditions at any time. Your continued use of the Site or Platform after revised Terms are posted constitutes your acceptance of those changes.

18. Disclaimer of Warranties

YOU EXPRESSLY AGREE THAT YOUR USE OF THE SITE AND PLATFORM IS AT YOUR SOLE RISK. THE SITE AND PLATFORM ARE PROVIDED ON AN "AS IS" AND "AS AVAILABLE" BASIS.

To the fullest extent permitted by law, RevoraMD disclaims all warranties of any kind, whether express, implied, statutory, or otherwise.

19. Limitation of Liability

TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, IN NO EVENT WILL REVORAMD OR ITS OFFICERS, DIRECTORS, EMPLOYEES, PARENTS, SUBSIDIARIES, AFFILIATES, AGENTS, CONTRACTORS, OR LICENSORS BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, EXEMPLARY, OR PUNITIVE DAMAGES.

20. Indemnification

You agree to indemnify, defend, and hold harmless RevoraMD and its officers, directors, employees, agents, affiliates, licensors, contractors, and suppliers from and against claims, losses, damages, costs, and expenses arising out of or related to your violation of these Terms and Conditions or your misuse of the Site, Platform, or Services.

21. Geographical Restrictions

RevoraMD makes no representation that the Site, Platform, products, services, or materials described on the Site are appropriate or available for use outside the United States or in every state, territory, or jurisdiction within the United States.
